4L60E Teardown and Inspection - No Fourth Gear: I'm Stumped!

I tear down a 4L60E out of a 2006 Chevy Suburban and come upon some interesting things, including a super-stuck selector shaft, clutch stack-up oddity and a mysterious failure that lead to the customer losing 4th gear.
