India’s air accident investigation bureau reports that there were no significant faults with the plane. Its preliminary report into the disaster, which killed more than 270 people, says that both fuel control switches were turned off shortly after take‑off, causing the engines to lose thrust. The cockpit voice recordings reveal that one pilot asked the other why they had activated the switches cutting off fuel to the engines. The Boeing Dreamliner came down in a densely‑populated area less than a minute after clearing the runway, causing devastation and many deaths on the ground.
